Decomposing pharmaceutical expenditure of Korean national health insurance for 2009¡­2013

Abstract
During the period of 2009~2013, Korean pharmaceutical expenditure increased by 10.8%. However pharmaceutical expenditure fluctuated as various policies, such as price cut in 2012, was implemented in this period. This research used claims data of national health insurance of Korea to decompose the factors that caused the changes in pharmaceutical expenditure in this period and we draw some policy implications. Preceding researches on this topic explains the pharmaceutical cost changes with three factors; price, volume and drug mix. We categorized pharmaceuticals in detail according to the characteristics and we analyzed the causing factors with Laspeyres decomposing approaches. We found that price effect in this period decreased pharmaceutical expenditure by 20.5% and exiting drugs effect resulted in 4.4% decreases of the expenditure. However, it was offset by the new drug entry effect (18.9% increases), drug mix effect of existing drugs (10.2%), prescription size effect (5.7%), prescription volume effect (4.4%) to have the final expenditure of 10.8% increases. The most contributing therapeutical class in the expenditure was antineoplastic and immunomodulating agents(L), followed by nervous system drugs(N), cardiovascular system drugs(C). Price effect was strong in sensory organs drugs(S), followed by genito-urinary system and sex hormones drugs(G)
